Pursuant to clause 30(3) of the COVID-19 Public Health Response (Air Border) Order 2021 (“Order”), I, Dr Ashley Bloomfield, Director-General of Health, hereby specify the following to be satisfactory evidence for the purposes of clauses 14, 15 and 30 of the Order.

1. All COVID-19 test results (required to satisfy the negative COVID-19 test requirement) must contain the following information:
1. the full name of person tested; and
2. the date of birth of person tested and/or the passport number of the person tested; and
3. the date on which the COVID-19 test was conducted; and
4. the time the COVID-19 test was conducted; and
5. the result of the test; and
6. the type of test that was conducted (PCR, RAT or LAMP); and
7. if the COVID-19 test was testing for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) viral ribonucleic acid using polymerase chain reaction (PCR test):
1. the name of the company or laboratory at which the PCR test was processed; and
8. if the COVID-19 test was testing for antigens specific to SARS-CoV-2 (rapid antigen test or RAT) or testing for SARS-CoV-2 viral ribonucleic acid using loop-mediated isothermal amplification (LAMP):
1. EITHER: the name of the health professional that supervised the test, including either a letterhead or stamp confirming the supervisor’s name, occupation and employer
2. OR: the name of the pharmacy, laboratory, healthcare entity, telehealth service, community-based or airport-based testing station that supervised the test.
2. The evidence may be in paper or digital form.
3. The evidence of a COVID-19 test result for persons arriving from a Group 3 place (as set out in schedule 2 of the Order) must be provided by a laboratory authorised to perform testing for COVID-19 by the government in the country in which the test is administered.
4. The evidence must be in English, or accompanied by a certified translation into English.

The notices “COVID-19 Public Health Response (Air Border) Order 2021 Evidence of Pre-Departure COVID-19 Test” made on 4 March 2022 (New Zealand Gazette, 4 March 2022, Notice No. 2022-go776) and “COVID-19 Public Health Response (Air Border) Order 2021—Evidence of Pre-Departure COVID-19 Test” made on 25 February 2022 (New Zealand Gazette, 25 February 2022, Notice No. 2022-go673) are revoked.

This notice will come into force at 11.59pm on 16 March 2022.
