Pursuant to clause 30 of the COVID-19 Public Health Response (Air Border) Order 2021 (“Order”), I, Dr Ashley Bloomfield, Director-General of Health, hereby specify the following to be appropriate evidence for the purposes of clause 30 of the Order.

1. All COVID-19 test results (required to satisfy the negative COVID-19 test requirement) must contain the following information:
1. full name of person tested
2. date of birth of person tested and/or the passport number of the person tested
3. if the COVID-19 test was testing for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) viral ribonucleic acid using polymerase chain reaction (PCR test), the name of the laboratory at which the PCR test was processed
4. if the COVID-19 test was testing for antigens specific to SARS-CoV-2 (rapid antigen test or RAT) or testing for SARS-CoV-2 viral ribonucleic acid using loop-mediated isothermal amplification (LAMP), the name of the health professional that supervised the test
5. written confirmation of:
1. the date on which the COVID-19 test was conducted; and
2. the time the COVID-19 test was conducted to establish that the COVID-19 test as undertaken within the 48-hour period for the PCR test or 24 hour period for the RAT or LAMP prior to the person’s direct flight or connecting flight
6. written confirmation from the laboratory (for PCR test) or health professional of the type of test that was conducted.
2. The evidence may be in paper or digital form.
3. The evidence must include either:
1. the supervisor’s letterhead; or
2. a stamp that confirms the supervisor’s name, occupation, and employer.
4. The evidence must be in English, or accompanied by a certified translation into English.
5. The evidence of a COVID-19 test result for persons arriving from a Group 3 place (as set out in schedule 2 of the Order) must be by a laboratory authorised to perform testing for COVID-19 by the government in the country in which the test is administered.

This notice will come into force at 11.59pm on 6 March 2022.
