Pursuant to section 88(2) of the Substance Addiction (Compulsory Assessment and Treatment) Act 2017, I, Dr John Crawshaw, revoke the approval of

Dr Tejpal Singh

as the Director of Area Addiction Services (Area Director) for Waikato District Health Board (New Zealand Gazette, 6 November 2017, Notice No. 2017-go5848).

Further, pursuant to section 88(1) of the Substance Addiction (Compulsory Assessment and Treatment) Act 2017, I designate Dr Andrew Darby, Consultant Psychiatrist, of Hamilton as Director of Area Addiction Services (Area Director) for Waikato District Health Board.
