Pursuant to clause 30 of the COVID-19 Public Health Response (Air Border) Order 2021 (“Order”), I, Dr Ashley Bloomfield, Director-General of Health, specify the following to be evidence of compliance with the vaccination requirement (at clause 13 of the Order) for the purposes of clause 30 of the Order:
The evidence that a COVID-19 vaccine has been administered must include the following details:
- A record of having received one or more COVID-19 vaccines that shows the name of the affected person.
- The brand name of the COVID-19 vaccine or vaccines (or any other information that identifies the vaccine, such as the manufacturer/developer or the trade name) that the affected person received.
- The name of the government or organisation who administered the vaccine or issued the proof of vaccination.
- The date on which the affected person received the most recent dose of the COVID-19 vaccine.
- In the case of a multiple dose schedule, the number of a series of doses as well as the overall number of doses in the series.
I further declare that the evidence must be in the following form:
- The evidence may be in paper or digital form.
- The evidence may not solely amount to a quick response (QR) code or barcode without any associated text.
This notice comes into effect at 11.59pm on 27 February 2022.
