COVID-19 Public Health Response (Air Border) Order 2021—Evidence of Vaccination

Pursuant to clause 30 of the COVID-19 Public Health Response (Air Border) Order 2021 (“Order”), I, Dr Ashley Bloomfield, Director-General of Health, specify the following to be evidence of compliance with the vaccination requirement (at clause 13 of the Order) for the purposes of clause 30 of the Order:

The evidence that a COVID-19 vaccine has been administered must include the following details:

  1. A record of having received one or more COVID-19 vaccines that shows the name of the affected person.
  2. The brand name of the COVID-19 vaccine or vaccines (or any other information that identifies the vaccine, such as the manufacturer/developer or the trade name) that the affected person received.
  3. The name of the government or organisation who administered the vaccine or issued the proof of vaccination.
  4. The date on which the affected person received the most recent dose of the COVID-19 vaccine.
  5. In the case of a multiple dose schedule, the number of a series of doses as well as the overall number of doses in the series.

I further declare that the evidence must be in the following form:

  1. The evidence may be in paper or digital form.
  2. The evidence may not solely amount to a quick response (QR) code or barcode without any associated text.

This notice comes into effect at 11.59pm on 27 February 2022.
