Pursuant to Regulation 18 of the Education (Board Elections) Regulations 2000, I hereby approve the following attached nomination form and voting paper in English, Te Reo Māori, Tongan, Samoan, and Simplified Chinese, for use in the specified circumstances:

1. Parent Representative Nomination Form (Form B), for use when a board opts into the mid-term election cycle in a triennial election year.
2. Parent Representative Voting Paper (Form D), for use for elections for parent representatives when a board opts into the mid-term election cycle in a triennial election year.
3. Staff Representative Voting Paper (Form E), for use in all elections for staff representatives.
4. Student Representative Voting Paper (Form F), for use in all elections for student representatives.
5. Parent Representative Voting Paper (Form G), for use in elections for parent representatives, for combined boards.

Form B replaces Form 2, Form D replaces Form 6, Form E replaces Forms 7 and 10, Form F replaces Forms 8 and 11, and Form G replaces Form 9.

This notice takes effect on the date of publication.
