Pursuant to the New Zealand Public Health and Disability Act 2000 and the Crown Entities Act 2004, the Associate Minister of Health has appointed
Dale Bramley
as Chair of the Health Quality & Safety Commission, for a term of office commencing on the publication of this notice and ending on 14 November 2021; and appointed
Raewyn Lamb
as a member and Deputy Chair of the Health Quality & Safety Commission, for a three year term of office commencing on the publication of this notice; and appointed
Philomena Antonio
Jennifer Parr
as members of the Health Quality & Safety Commission, for a three year term of office commencing on the publication of this notice; and appointed
Shenagh Gleisner
as a member of the Health Quality & Safety Commission, for a three year term of office commencing on 31 November 2019; and appointed
Peter Crampton
as a member of the Health Quality & Safety Commission, for a three year term of office commencing on 1 April 2020.
Note: This notice replaces the notice published in the New Zealand Gazette, 2 October 2019, Notice No.2019-go4579, which incorrectly stated that the appointments were made pursuant to the Health Practitioners Competence Assurance Act 2003.
