Under section 140 of the Intellectual Disability (Compulsory Care and Rehabilitation) Act 2003, I revoke the appointment of the following person as a compulsory care coordinator for the named geographical and operational area listed below, as published in the New Zealand Gazette, 1 May 2014, No. 44, page 1290.

Northern region, being the geographical area including Auckland and Northland

The revocation of this appointment takes effect on the date this notice is published in the New Zealand Gazette.
