Pursuant to section 24(7)(b) of the Misuse of Drugs Act 1975, I, John Crawshaw, acting under delegated authority from the Minister of Health, specify the Christchurch Opioid Recovery Service as a service that may provide treatment with controlled drugs for people who are dependent on controlled drugs at the following place:
Hillmorton Hospital
Sylvan Street
Christchurch 8024
And, pursuant to section 24(7)(a) of the Misuse of Drugs Act 1975, acting under delegated authority from the Minister of Health, I specify
Dr Carmen Ruth Lowe
as a medical practitioner, may:
- prescribe, administer or supply controlled drugs for the treatment of dependence to controlled drugs as the lead clinician for the Christchurch Opioid Recovery Service;
- authorise in writing service/clinic medical practitioners as specified under subsection (2)(b) to:
- prescribe, administer or supply controlled drugs for the treatment of dependence;
- authorise in writing general practitioners receiving clients from the Christchurch Opioid Recovery Service as specified under subsection (2)(d); and
- authorise in writing general practitioners receiving clients from the Christchurch Opioid Recovery Service as specified under subsection (2)(d).
Conditions
- Controlled drugs may only be prescribed, administered or supplied to people who are or have been clients of the Christchurch Opioid Recovery Service or the Pegasus Health GP Care programme.
- This approval expires on 31 December 2017.
