Pursuant to section 24(7) of the Misuse of Drugs Act 1975, I, John Crawshaw, acting under delegated authority from the Minister of Health, revoke the approval of the Waikato District Health Board Alcohol and Drug service and medical practitioner issued under this section on 3 July 2014 (New Zealand Gazette, 3 July 2014, No. 70, page 1969).

This revocation will take effect on 14 September 2015.
