I appoint

as a commissioner from 12 June 2013 to 31 May 2016, and as chair from 1 August 2013 to 31 May 2016; and
Roger Heydon Bell
as a commissioner from 1 August 2013 to 30 June 2015; and
Dr Alison Claire O'Connell
as a commissioner from 1 September 2013 to 30 June 2015; and reappoint
Russell John Black
as a commissioner from 1 July 2013 to 30 June 2016; and
Trevor John Burt
as a commissioner from 1 July 2013 to 31 December 2013.
