Pursuant to section 52(1) of the New Zealand Public Health and Disability Act 2000 and section 28(1)(a) of the Crown Entities Act 2004 ("the CE Act"), and notice given under section 28(2) of the CE Act, I have appointed

as a member of the Pharmaceutical Management Agency Board for a term of office commencing on the date of this notice and ending on 31 July 2015.
