Pursuant to section 9(1) of the Health and Disability Commissioner Act 1994, and section 28(1)(b) of the Crown Entities Act 2004, the Governor-General of New Zealand, on the recommendation of the Minister of Health, has appointed
Theodora Mary Baker
as a Deputy Health and Disability Commissioner for a term of five years commencing on 25 July 2011 and expiring on 24 July 2016.
