Pursuant to section 37 of the Health and Disability Services (Safety) Act 2001 ("the Act"), I, Mark Jacobs, Acting
Chief Medical Officer, Clinical Leadership, Protection and Regulation, Ministry of Health, under delegation from the Director-General of Health, cancel the designation of
Telarc SAI Limited
to audit the provision of health and disability services under the Act.
This notice takes effect from 1 April 2011.
