Corrigendum—Provisional Consent to the Distribution of a Changed Medicine

In the notice published in the New Zealand Gazette, 4 June 2009, No. 79, page 1859, the conditions for Thalidomide Pharmion are to be deleted and the following substituted:
“? Thalidomide may only be prescribed by registered medical practitioners as defined in the Health Practitioners Competence Assurance Act 2003 who are certified as competent by the Medical Council of New Zealand in the scope and practice of internal medicine.
? Douglas Pharmaceuticals Limited shall manage the risk management programme relating to the distribution and use of Thalidomide.”
