Pursuant to section 29(1)(b) of the New Zealand Public Health and Disability Act 2000, I appoint, under section 28(1)(a) of the Crown Entities Act 2004
Lester Levy
as a member of the board of Waitemata District Health Board; and, pursuant to clause 10(1) of Schedule 3 to the New Zealand Public Health and Disability Act 2000, I also appoint
Lester Levy
as chairperson of the board of Waitemata District Health Board.
These appointments will take effect on 5 June 2009 and end on 4 June 2012.
