Under section 140 of the Intellectual Disability (Compulsory Care and Rehabilitation) Act 2003, I revoke the appointment of the following person as a compulsory care co-ordinator for the named geographical and operational area listed below, as published in the New Zealand Gazette, 9 November 2006, No. 132, page 3829.

The revocation of this appointment takes effect on the date this notice is published in the New Zealand Gazette.
