Pursuant to section 28(1)(a) of the Crown Entities Act 2004, I appoint to the New Zealand Blood Service Board

as members; and, pursuant to sections 28(1)(a) and 32(2) of the Crown Entities Act 2004, I reappoint
Anne Urlwin
as a member; and, under clause 1(1) of Schedule 5 of the Crown Entities Act 2004, I also appoint
Anne Urlwin
as chairperson.
These appointments, for three-year terms of office commencing 15 August 2008, were made by way of written notice to the members (with a copy to the entity) pursuant to section 28(2) and clause 1(1) of Schedule 5 of the Crown Entities Act 2004.
