Pursuant to section 29(1)(b) of the New Zealand Public Health and Disability Act 2000 and notices given under section 28(2) of the Crown Entities Act 2004, I have appointed or reappointed as members of the board of
the district health boards ("DHB") listed in Column 2 of the table below, the persons named in Column 1 of that table whose name appears alongside the name of the DHB.
The appointment or reappointment of those persons as members takes effect on 28 April 2008 and will end on
5 December 2010.
