for a two-year term of office commencing 17 December 2007; and appoint

for a three-year term of office commencing from the date of this notice; and appoint
Dr Nicholas Baker
as chair to the Child and Youth Mortality Review Committee for a term of office commencing 17 December 2007 and ending 28 February 2010.
