Member Appointments/reappointments
Pursuant to section 29(1)(b) of the New Zealand Public Health and Disability Act 2000 (“the NZPHD Act”) and notices given under section 28(2) of the Crown Entities Act 2004, I have appointed as members of the board of the district health boards (“DHBs”) listed in Column 2 of the table below, the persons named in Column 1 of that table whose name appears alongside the name of the DHB.
The appointment of those persons as members takes effect on 17 December 2007.
The appointments end on the date which appears in the relevant row of Column 3 of the table below, alongside the name of the DHB that those persons have been appointed to.

Chairperson and Deputy Chairperson Appointments/reappointments
Pursuant to clause 10(1) of Schedule 3 to the NZPHD Act, I appoint as deputy chairpersons and chairpersons of the board
of the DHBs listed in Column 2 of the table below, the persons named in Column 1 of that table whose name appears alongside the name of the DHB.
The appointment of those persons as chairpersons and deputy chairpersons takes effect on 17 December 2007.
The appointments end on the date which appears in the relevant row of Column 3 of the table below, alongside the name of the DHB that those persons have been appointed to.
