Pursuant to section 9 (1) of the Health and Disability Commissioner Act 1994 and section 28 (1) (b) of the
Crown Entities Act 2004, the Governor-General, on my recommendation as responsible Minister, has appointed
Rae Lamb
as Deputy Health and Disability Commissioner, Complaints Resolution.
This appointment will take effect on 1 August 2005 and will end on 1 August 2010, and was made by way of written notice to the Deputy Health and Disability Commissioner, Complaints Resolution (with a copy to the entity), pursuant to section 28 (2) and clause 1 (2) of Schedule 5 of the Crown Entities Act 2004.
