Optometrists and Dispensing Opticians Fees

Notice 2004
Pursuant to section 130 (1) of the Health Practitioners Competence Assurance Act 2003, the following notice is given.
N o t i c e
1. Title and commencement-(1) This notice may be cited as the Optometrists and Dispensing Opticians Fees Notice 2004.
(2) This notice comes into force on 18 September 2004.
2. Fees-The Opticians Board* sets the following fees payable to the board specified in attached Schedule.
3. Tax-The fees are inclusive of Goods and Services Tax.

Application for registration:
For New Zealand and Australia trained optometrists and dispensing opticians 230.00
Under the Trans-Tasman Mutual Recognition Act 1997 230.00
For overseas (excluding Australia) trained optometrists and dispensing opticians 550.00
Practising certificate fee for practitioners on the Registers of Optometrists and Dispensing Opticians holding a practising certificate ina year 1 April to 31 March who purchase a certificate for the following year prior to the expiration of their current practising certificate (up to and including 31 March) 280.00
Practising certificate fee for practitioners on the Registers of Optometrists and Dispensing Opticians holding a practising certificate in the year 1 April to 31 March who purchasea certificate for the following year after the expiration of their current practising certificate (after 1 April) and who are still practising after 1 April 420.00
Change of scope application 115.00
Any certificate or copy of any certificate 30.00
Restoration of name to the Register 55.00
Supplying any document to enable registration outside New Zealand 30.00
Copy of Register 30.00
