Appointments/reappointments to District Health Boards

Member and Chairperson Appointments/reappointments
Pursuant to section 29 (1) (b) of the New Zealand Public Health and Disability Act 2000 ("the NZPHD Act"), I appoint as members of the board of the District Health Boards ("DHBs") listed in Column 2 of the table below, the persons named in Column 1 of that table whose name appears alongside the name of the DHB.
Pursuant to clause 10 (1) of Schedule 3 to the NZPHD Act, I appoint those members as chairpersons of the DHB to which they have been appointed.
The appointment of those persons as members and chairpersons takes effect on the date which appears in the relevant row of Column 3 of the table below, alongside the name of the DHB to which those persons have been appointed.
The appointments end on the date which appears in the relevant row of Column 4 of the table below, alongside the commencement date.

Chairperson Appointments/reappointments
Pursuant to clause 10 (1) of Schedule 3 to the NZPHD Act, I appoint as chairperson of the board of the DHBs listed in Column 2 of the table below, the persons named in Column 1 of that table whose name appears alongside the name of the DHB.
Each appointment as chairperson is for the duration of the person's forthcoming term as an elected member (that is, commencing on 6 December 2004 and ending on 9 December 2007).

Pursuant to section 29 (1) (b) of the NZPHD Act, I appoint
Ross Barry Keenan
as a member of the boards of Waitemata DHB, Auckland DHB and Counties Manukau DHB.
Pursuant to clause 10 (1) of Schedule 3 to the NZPHD Act, I also appoint
Ross Barry Keenan
as deputy chairperson of the boards of Waitemata DHB, Auckland DHB and Counties Manukau DHB.
These appointments take effect on 6 December 2004 and end on 5 December 2007.
