The Registrar of Incorporated Societies is satisfied that the declaration of dissolution dated the 2nd day of October 1997 of Pacific Community Health Incorporated
WN. I.S. 616074 was made in error and on the 29th day
of September 2000, made a declaration of revocation of dissolution as from that date.
