19 September 2004, designated
the manager, Medsafe
the team leader, Medicines Control
as licensing authorities for the purposes of the Medicines Act 1981.
Applications for a licence must be made in the prescribed form to the Licensing Authority, Medsafe, Ministry of Health, P.O. Box 5013, Wellington.
