Pursuant to section 24(7)(a) of the Misuse of Drugs Act 1975, I, John Crawshaw, acting under delegated authority from the Minister of Health, specify that Dr Zelda Strydom may:
- prescribe, administer and supply controlled drugs for the treatment of dependence, as the lead clinician for Bay of Plenty Opioid Substitution Service;
- authorise in writing service/clinic medical practitioners as specified under section 24(2)(b) of the Misuse of Drugs Act 1975 to:
- prescribe, administer or supply controlled drugs for the treatment of dependence;
- authorise in writing general practitioners receiving clients from Bay of Plenty Opioid Substitution Service as specified under section 24(2)(d) of the Misuse of Drugs Act 1975; and
- authorise in writing general practitioners receiving clients from Bay of Plenty Opioid Substitution Service as specified under section 24(2)(d) of the Misuse of Drugs Act 1975.
Conditions
- Controlled drugs may only be prescribed, administered or supplied to people who are or have been clients of Bay of Plenty Opioid Substitution Service.
- This approval expires on 31 December 2020.
